Children with cancer may benefit from proton treatment because of precise tumour targets, limited radiation to surrounding structures and reduced immediate and long-term side effects. A new Swedish study by Asklid et al. investigates reasons for choosing alternative radiotherapy modalities in children. Overall, protons accounted for one-third of the treatment courses. The most common reasons for choosing alternative modalities were non-curative intent, lack of superior expected outcome and use of total body irradiation. The study demonstrates how medical, technical and practical considerations need to be balanced for personalized radiotherapy in the childhood population. Head and neck cancer has a strong link to well-known risk factors such as tobacco, alcohol, and human papillomavirus (HPV). While lower education is generally associated with a higher risk of head and neck cancer, higher education appears linked to HPV-related cases. Nikkilä et al. report lower incidences of several subtypes of head and neck cancer in rural populations. However, the rates of oropharyngeal and oral cavity cancer are increasing in both sexes, across all education and urbanization levels, particularly since the start of the century.
